Are there good templates available for tracking and documenting palliative care service interventions?

We've used two instruments: One to track symptom burden and the effect of pc on symptoms - this is completed daily by the team (CAPC clinical rounding tool). Additionally, we have another form that is completed at the end of the consult by the team that documents exactly what the team did. I would be interested in seeing the 2nd tool. I am using the edmonton symptom scale to measure pain and symptom management, but I have not seen anything that really says what I did...All the long talks with families facilitating decision-making etc... Some things are hard to measure- too subjective.

I like your idea of using a separate tool to review the case at the end of service. I too have been wondering how to capture all that we do for our patients. I would be interested in seeing the second form you use, thank you.
