Post P&P for automatic referrals for readmissions

Does anyone have a p&p they are willing to share addressing automatic consults on hospital readmission for patients previously seen by the PC team? I am hoping to implement this in our hospital and avoid delays in getting reconsulted. Thanks

I don't have a P&P for automatic consults but you raise an interesting point. Yes, it can be very valuable to follow-up when patients are re-admitted to the hospital. There are some hospitals that have hard wired Palliative Care involvement and advanced directives to the registration process. I think the first task is identifying where the trigger would be generated from. If you have a physician on the team the trigger could be a prompt to the attending physician that Palliative Care had involvement in the past and if there was interest in having the physician follow-up and see the patient an order could be written for the consultation. A physician order is required so that the Palliative Physician can bill for the visit. An order would not necessarily be needed for other team member involvement depending on the sturcture and processes your team and system have established. You may want to talk with your IT connection and see if there is a similar prompt for other services and then work through the system to see if you could get something similar in place for a PC prompt. It's a great idea but may not be as easy as it sounds--unfortunately.
