Hello colleagues!

Our hospital (like many others) is in the midst of transition to an Electronic Medical Record. Our Palliative Care team is using this transition as an opportunity to expand and improve our statistical tracking. I've been asked to suggest categories or values that we may want to track from the spiritual care standpoint. Do any of you currently do this kind of record keeping? If so, would you share what you use and how it works? I am imagining using a spiritual assessment that would include values we would want to track. I would be VERY grateful for any guidance and experience in this area.

All chaplain visits and initial assessments are documented in the EMR but currently cannot track numbers of visits or initial assessments. This is done by the chaplaincy department separately.

The problem here for chaplains is that we don't yet have a good set of accepted outcomes against which to measure. Most people I know are using process measures at least for the moment. By example, if all palliative care patients are supposed to get a spiritual screening when referred to the service, what percentage of the time does that happen? If the chaplain is supposed to visit all newly referred patients, how often does that happen? Are assessments actually being documented? If you have a service in which there is an outcome specified like determining the presence of Advance Directives, that can and should be tracked.

Developing outcomes is a major goal for chaplaincy generally. It would be very helpful if anyone has piloted some outcomes, if they would share what they have done.
