Hello colleagues!

Our hospital (like many others) is in the midst of transition to an Electronic Medical Record. Our Palliative Care team is using this transition as an opportunity to expand and improve our statistical tracking. I've been asked to suggest categories or values that we may want to track from the spiritual care standpoint. Do any of you currently do this kind of record keeping? If so, would you share what you use and how it works? I am imagining using a spiritual assessment that would include values we would want to track. I would be VERY grateful for any guidance and experience in this area.
