Our team uses the hospital-wide self-developed tool which is undergoing revision but essentially is a check-list of assessment domains (i.e. Forgiveness, Isolation, Hopelessness etc...) , interventions (i.e. Life Review, Bereavement care, Prayer etc..) and outcomes (i.e. Enhanced sense of meaning, Enhanced sense of self, etc...).
