Post Bereavment materials Post INpt PC

I am wondering if anyone has Palliative Care specific Bereavment materials that they are
sending out after someone has died on PC service while inpt ( hospital), and if they would be willing to share information about sources. Thanks!

Our system has a bereavement booklet that is sent out to families after a death. We also send out a sympathy card to families of patients who died on the PC unit. I'm curious what you are thinking would need to be specific to PC for these materials. Most likely you would be able to use materials that already exist in your system in chaplaincy or could be provided by a local hospice program. Here at Dartmouth Hitchcock Medical Center we send out a card signed by the members of the palliative care service that worked with the patient and family. Messages are personalized. We cards that have photographs on the front. We use a clear sticker that has wording from our service. We hand sign and include our personal messages. We celebrate a bereavement ritual for the members of the service once a week. We have a team of trained bereavement volunteers that call the family of the deceased within two weeks of the death. Family/significant others, etc. are invited to a service of remembrance held four times a year at the medical center. We do not send out any type of brochures or handouts although there are plenty of that type of thing available. Families appreciate the hand written notes and tell us so.
