Post Chaplains as Leaders of Palliative Care Teams

Doing some research for our organization. I am interested in hearing from Instiutions/Chaplains where the Chaplain serves as Team Leader for a Palliative Care Team. Or where they may be assigned as Lead Team member for a particular patient.

I know this post is old. I was wondering if you ever got any replies. I have begun a palliative care team, which at this point is a consult service. I am pastoral care director. We hope to continue to build the service and expand what we do.
