Post Chaplains as Leaders of Palliative Care Teams

Doing some research for our organization. I am interested in hearing from Instiutions/Chaplains where the Chaplain serves as Team Leader for a Palliative Care Team. Or where they may be assigned as Lead Team member for a particular patient.
