I am a hospice chaplain and I am in the process of designing a new assessment tool to use with our patients. I would appreciate any of you that uses an effective tool to send me a copy. Thanks for your help on this project.

It would be important to be specific as to who is going to use this tool and for what purpose. Is this something any member of the team would use upon admission (i.e. a screening tool), a tool a nurse or physician would use to take a spiritual history, or a tool to be used by the chaplain to whom the patient has been referred (i.e. an assessment tool)?
