It would be important to be specific as to who is going to use this tool and for what purpose. Is this something any member of the team would use upon admission (i.e. a screening tool), a tool a nurse or physician would use to take a spiritual history, or a tool to be used by the chaplain to whom the patient has been referred (i.e. an assessment tool)?
