I'd encourage you to look at the NHPCO (National Hospice and Palliative Care Organization)website. There are lots of resources there including the medicare conditions of participation. This outlines requirements of a hospice program in terms of essential elements on the interdisciplinary team. NHPCO also has some focus groups for the different disciplines.
