Matthew Loscalzo, MSW, now at City of Hope, who is highly recognized authority on psycho-social-spiritual screening in oncology strongly advocates for some kind of screening on first contact with the institution. That usually means in the clinic. Note that he is advocating for a screening, not an assessment. Matt has pioneered some electronic methods where patients use a computer touch pad to fill out the screening while they are waiting. The data transfers automatically to the doctor's office computer. There should be some set of spiritual triggers in the assessment which generate a chaplain referral automatically. The assessment can then follow.
