I believe that asking religious history/denomination can be very helpful. It gives an understanding of where folks "came from" even if that has changed. Also, 15 years ago or so there were a lot of formal spiritual assessments. The one I like best was published in Health Progress magazine in 1993. It allows for a self-assessment. I have a hard copy - will get it entered so it will be available to anyone who would like it.
