I'm director of pastoral care in a norther NJ for profit hospital and sit on the palliative care and ethics committees. I would appreciate a copy of the assessment tool as well.

How do I get a copy David? Here in my country Cameroon hospice program is just still on a dew feet and I happen to be the first chaplain on the site. There are no materials any where yet for use but I hope findind this web site is a blessing to me. Please a copy of that assessment tools will help me alot.
