Post what modifier to use for Medicare

We have a question about the Q4 modifier ?

Is this modifier ever required when Medicare is billed for Palliative care ?

e.g. our provider bills CPT 99232 with a primary diagnosis of 799.3

Does the Q4 modifier have to be appended to the claim ?

When would the 77 modifier need to be used ?

I've really done some digging on the Q4 modifier and have come up with very little myself. What I've found is that the Q4 is an informational modifier only and may be submitted with codes 80002-89399 and G0058-G0060. Service for referring/ordering physician qualifies as a service exemption.Based on that little bit of information it doesn't appear to be an appropriate modifier or a required modifier for PC services.
