Post what modifier to use for Medicare

We have a question about the Q4 modifier ?

Is this modifier ever required when Medicare is billed for Palliative care ?

e.g. our provider bills CPT 99232 with a primary diagnosis of 799.3

Does the Q4 modifier have to be appended to the claim ?

When would the 77 modifier need to be used ?
