Post Hospice medical director contract

I am an internist. About 9 months ago I agreed to volunteer as the hospice medical director in the small county where I live. We typicaly have about 10-12 patients on hospice at any one time. My responsibilities include going to a monthly meeting, signing careplans, being available 24/7, and occasional home visits. I do about one visit every month or two. I am not doing this for the money. I was a nurse for many years before I started medical school, and I always felt drawn to hospice patients. Yet, I do believe I should be paid something for my labor. Up until now I have been paid zero. What should I ask for my responsibilities here? I am not certified, but I would like to be. I am, however a creative thinker on how to manage pain and other medications and quite honestly the best physician for this job in my little county because I have the disposition to deal with these patients and their families.
