As to #4. palliative care visits in outpatient setting: we receive these patients as a consultation from another physician. Medicare requires documentation that the consultation was requested, as well as documentation back to the physician who requested the consult as to your recommendations or treatment. We use 99241-99245 for these visits (initial). Any visits (outpatient) with the patient after that are billed using 99212-99215. The four r's of consultation are request, reason, render and report. Medicare needs to see documentation of all four in the clinical chart.
