It differs fom payer to payer, and from state-to-state.
Are you referring to the social worker's visits to the patients' homes? To patients in the hospital? To both?

This would be for visits to an outpatient clinic. She is not connected with homecare or hospice. Thanks...I appreciate any help with this!

We are in a tristate region (OH, KY, and IN) however, our Social Worker is seeing people from our outpatient clinic area. Thanks for your help!! Mary
