Great question--there this no magic formula for this-but clearly, the program needs to allocate time for a Program Director to manage quality initiatives, budgets, marketing, education, community liasons, etc. This position does not have to be a physician. For a start-up service, 0.2 FTE would be my minimum recommendation.

From our experience and study of palliative medicine compensation, we've found that compensation for "administrative" responsibilities is often reverse engineered, that is to say, it's backed into after establishing a performance metric for the provision of clinical services. The other key question, of course, is the "hourly rate" to be paid for administrative activities. As one might expect, this is typically part of the negotiations between the program sponsor and physician/group.
