Post Reimbursement Based on Acuity Level

I am developing a home-based palliative care program and we are considering creating a Level of Care system that is based on acuity (similar to the hospice model). This level of care would then determine the frequency of services the beneficiary is entitled to.

Does anyone else do a similar model?
