Post Reimbursement Based on Acuity Level

I am developing a home-based palliative care program and we are considering creating a Level of Care system that is based on acuity (similar to the hospice model). This level of care would then determine the frequency of services the beneficiary is entitled to.

Does anyone else do a similar model?

There are some home care acuity systems that are available. Although I haven't personally used them I would imagine that they would address the needs of patients that you would be seeing at home with palliative care. We are looking forward to CAPC's collaboration to develop another product that will be premiering this fall at the CAPC conference this November in Miami. It will be IPAL Outpatient.
