There are some home care acuity systems that are available. Although I haven't personally used them I would imagine that they would address the needs of patients that you would be seeing at home with palliative care. We are looking forward to CAPC's collaboration to develop another product that will be premiering this fall at the CAPC conference this November in Miami. It will be IPAL Outpatient. It is a follow-up to the IPAL initiatives. Check out the ICU and ED IPALS at ttp://www.capc.org/ipal.
