Is it ok to bill an element based visit (99222) which stated 50 min., along with 99356 for a family meeting regarding same patient later that day that lasted 60 min. Or...when billing prolonged codes, do you have to stay time based on both parts of the visit? Do you have to bill highest visit 99223/33 in order to then tack on an extended visit code if applicable? Thank you.

Who is the carrier? Was the patient present during the family meeting?
