Post CPT Modifiers for PC - Phys vs. NP?

We are new to Palliative Care billing, waiting for our clearance from Medicare to begin billing. However, we're stuck on the billing for physicians versus nurse practitioners. The documentation states that Medicare will pay 100% for physicians and 85% for nurse practioner visits - but how do we indicate on the claim who sees the patient? Since the CPT code may be the same, and the claim can only show 1 physician/nurse practioner...we're drawing a blank on how to show the difference in WHO sees the patient on the claim. Any insight or documentation for these procedures would be greatly appreciated, thanks!
