Your concern about using the V66.7 code is valid. We have found that when this code is applied by billers it generally does not reliably indicate patients that have been seen by the PC service. When the services are billed we have the clinical staff indicate the top 4 symptom diagnosis and then the coder/biller adds the V code as a supplemental code. It is important to keep a list of patients that you see so that you can take credit for what you are doing to transition the plan of care and for cost savings but are not being held accountable for patients that the service has not seen.
