Post Medicare Part B Billing Application

We are a hospice provider who is developing a Palliative Care Program and must complete the application for Part B. Does anyone know which Medicare Application is the correct one. Our Medical director is Certificed in Hospice and Palliative Care and will oversee the program and our NP will perform the consult visits. Is the correct form CMS -855B or CMS 855I or neither one. If anyone can help I would appreciate it.
