I notice there has been a fair amount of discussion concerning the billing of Med B for services rendered by a licensed CSW. Does anyone have a sample billing form they would be willing to share. I'm trying to produce something for our LCSW.

With respect to billing issues, I have found the following document quite helpful:

We have billed for a CSW in the past, however all of her services were billed via EMR. I have many other encounter forms, but not one specific to CSW; sorry.
