Post associate medical director billing

we are new to having a medical director part time. he wants to be able to visit every patient on service prior to the recert at the 3rd period to get a look at the patients condition in order to have a reference point. but if he finds an issue unrelated to the terminal diagnosis, and "consults" with the attending on the case is he able to bill separately for that visit and how would he do so? we do not bill part b medicare at all only part a hospice.
