Does anybody know if there are any codes to bill for discharge planning and goal setting for palliative care in the inpatient hospital setting?

There aren't any ICD-9 codes that can be used for goal setting/goals of care or discharge. I would suggest that you use either the disease itself or signs/symptoms that you are managing.
