Is it required or common practice to obtain a signed consent from the patient at the time of a palliative consultative visit (in hospital, ECF, AL or home)?

We were advised by a extreamly reliable source...yes. For out patient, NH, ALF and so on. For the in patients, we were told, we are covered under the Hospitals-so no for them only.
