The decision to decline enrollment under the Medicare Hospice Benefit for a patient receiving radiation is a financial decision by the hospice agency, not a regulatory issue. Under the MHB, a hospice is responsible for payment for all palliative treatments within the plan of care; if the hospice had enrolled the patient, the hospice would have to pay for the radiation, an expensive proposition, from the daily per diem payment(~$150/day). Thus, many hospices will decline to enroll during radiation, waiting until the radiation course is completed before enrollment. There is no separate palliative care payment system apart from the Medicare Hospice Benefit.
