I am a chaplain on a team and I usually write the team prog. note in the pt. record. Our MD also writes a consulting note that includes medical and other recommendations. My note discusses nature of coversation with pt. and family as well as goals and plans that have been agreed upon for the referring physician's information.
Coder came to me and asked if she could use ICD9 V code to report the activity based on my note. That is my question to you.

I believe that Coding may only code based on physician documentation. If there has been some change in standard, I'm not aware of it.

I agree that the facility coder should not be using the docmentation of a chaplain to assign any diagnosis code to the patient's hospital stay.
