Post Hospice Billing in Nursing Home

Hi, I am trying to find out how to bill a patient who is in hospice but resides in a nursing home. I have seen CPT code 99309 - 99310 billed with modifiers but I do not know if that is accurate. Can anyone give me information regarding this? Thank you so much

Initial Nursing Facility Care (admit)
99304-99306 (the documenatation for these codes is similar to initial inpt codes 99221-99223. Subsequent nursing facilty care codes 99307-99310; the documentation requirements for these codes are similar to established visit codes 99211-99215. If you are the hospice attending and have a financial contract with the hospice agency you send a claim to them. If you're the hospice attending without any financial arrangement you add the GV and bill Medicare.
