As a family physician and PMed specialist, are there any codes I can use to bill for pscyhological counseling? Given the lack of readily available psychological and psychiatirc services, I frequently have to manage depression and anxiety. In addition, I frequently manage difficult family dynamics that impact on patient care. What about CPT codes 90846 abd 90847? Thank you. The advice offered on this listserv is invaluable.

Before you start billing for family psychotherapy, make sure you read the CMS policy. http://www.wpsmedicare.com/part_b/policy/active/local/_files/l30489_psych014_cbg.pdf

There are very specific requirements and only certain diagnoses will be covered by CMS. If you're going to deal with commercial carriers, that too can be problematic as services may be covered under the patients mental health policy which is usually a different carrier than the medical carrier. We currently have a social worker billing those codes in our Geriatrics clinic and it's been very educational.
