Does it make a difference whether you bill 2 units of 99357 on one billing line or bill 1 unit on two separate lines?

I believe that we have always billed two lines; with one line and quantity of two some payers only reimburse you for one. Unless the payer specifically tells you to use one line with the quantity of two, I'd stick with two separate lines.
