Post PM consult in Hospice

I have presented this problem in the past but need clarification and specific help with this issue. We, a board certified palliative medicine MD and NP, have a consulting service in a community hospital. Currently we may assume primary care for a patient in the inpatient hospice unit that is attached to the hospital. We contend that if the primary doc wants to remain the physician of record in hospice and consults us, the palliative medicine team, to manage pain and symptoms, that should be in line with Medicare. We bill for symptoms, etc. The hospice program contends that we cannot see our patients in consult in the IPU. The hospice medical director is not board certified and serves in a administrative role only. Any comments, advice, expert opinions.
