Post Reporting structure for PC Consultation Program

Hello, All-

We currently have an inpatient Palliative Care Consultation program (4 months old), and are looking to branch out to other venues. Our health system owns a VNA. We would like to bring the consultations to homecare patients, local SNFs, and outpatient Radiation Oncology and Surgical patients. (Radiation Oncology and Surgical Services have separate physician practices outside the Health system).

Currently, my NP and MD salary is split 50/50 between the VNA Hospice Department, and a specialty service department at the hospital. In order to be able to bill for all the above-mentioned services, which department of the health system should "own" the Palliative Care Program? I am very interested to hear from any of you regarding the reporting structure of your Palliative Care program.

There is no one right department that should "own" palliative care. It is based on your organization and your goals. In hospital systems across the country you will find it located in administration, medicine, hospice etc. You didn't mention which "specialty service dept." that the NP/MD salary split is between. That might be another option. Whichever dept you decide upon, you'll need to get the Part B provider status for the providers so they are able to bill for their services whether they are going out to your homecare patients, to SNFs or to other clinics as a value added service.
