We are starting an outpatient palliative care program.
Would any one be willing to share their current outpatient billing form/router?

Hello,
There are examples of billing forms from different palliative care programs on the CAPC website, under "tools for palliative care programs."
Please let me know if you do not find what you need, and whether you are interested in outpatient clinic billing or other outpatient settings.
