We have only been able to bill for social work services in the outpatient clinic setting, similar to how a psychologist would bill for professional services. We have a LICSW who works with a palliative physician or nurse practitioner in our palliative care clinic. She is credentialed with the payors. She is not required to have a collaborative practice agreement. I know there are differences among states related to licensure for social workers, and the state board for social work may be able to clarify specific regulations in your state.
In the home setting, there may be a palliative care benefit within some health plans that would provide reimbursement for advance care planning in the home--there are a few in this state. The providing agency would receive payment for this service--usually involving a nurse and social worker/chaplain--apart from direct social work billing.
