Post Billing in the Hospice IPU

Our palliative medicine service is asking the Hospice Medical Director to admit our patients on the weekend and our PC service will assume the patients on Monday. We do not have the capacity to see patients 24/7. Do we bill on Monday for a new admission or FU visit?

There are a couple of questions that we would need you to clarify: where are the patients being admitted to and are they being admitted as acute care or GIP patients? In general it would seem that the person who is admitting the patient would bill for the admission and the subsequent visits on Monday would be FU. You will also want to be cautious that you have a way to differentiate the cost reporting for the billed services. We have two cost centers, 1 for Acute Palliative Medicine Service and one for Hospice. They both have very different billing guidelines.

Since it is not a new admission your providers
would bill subsequent visits.

PER CPT: For initial inpatient encounters by
physicians other than the admitting physician
, see initial inpatient consult codes (99251-99255) or subsequent hospital care codes (99231-99233) as appropriate.

Since your providers know that they are assuming care, they should never use the consultation codes.
