There are certainly independent hospice & palliative care medical groups out there, whose work pre-dated the hospital-based palliative care movement but now includes that as one component. Three of the largest which I'm aware of are Capital Hospice in northern VA (http://www.capitalhospice.org/), Hospice & Palliatice Care of the Charlotte (NC) Region (http://www.hospiceatcharlotte.com/index.cfm), and Hospice of the Bluegrass in Lexington KY (http://www.hospicebg.com/). Each of them contracts with local hospitals as well as providing hospice care. Let me know if this helps or not.
