I received additional information on outpatient social work billing from Susan Swinford at Hospice of the Bluegrass:

"We bill our LCSW for outpatient and can also bill for home patients-
there are codes for social work Part B billing- The SW has to use a DSM III R diagnosis- the one we use most is Adjustment Disorder. "
