Post Reimbursement for social work from Medicare Part B

Does anyone successfully bill Medicare Part B for clinical social work and gotten paid? What is the primary diagnosis code to use for billing, the terminal illness or the "mental health" diagnosis code? Thanks for the help.

I would also appreciate this information. We are in the process of hiring a social worker for our PC program and are trying to figure out how and if she/he can bill. Thank you. Mary
