Regarding billing for consult services in the context of a palliative care consult service, do hospitals ever do the billing for physician services or are those typically done by the physician independantly. I was wondering if that revenue was used to offset medical director expense or purely as physician revenue.

There is considerable variation--depends on the physician practice--hospital financial relationship. So the answer is all the above are options.

There are numerous models on how physicians are compensated for the services they provide to palliative medicine programs, as the Weissman post points out. All models should be considered when a sponsor is organizing (or reorganizing) a palliative care program - as one might expect, each model offers advantages (and disadvantages, of course) in each particular situation, depending upon local characteristics. If one closely examines other specialties whose primary service is to hospital inpatients, one is likely to find that the dominant model(suggesting that the model has stood the test of time) is that where the practitioner contracts independently with the hospital to provide professional services (and thus bills the payers directly) and is compensated a mutually agreed upon amount by the hospital (or other program sponsor) for other resonsibilities, such as building goodwill and monitoring quality.
