About The Web Conference Event

In 2011, Presbyterian Health System in New Mexico launched outpatient palliative care clinics in three primary care sites – each a Patient Centered Medical Home. The goal of these clinics is to reach patients with serious illnesses and debility much earlier in their disease course. The clinical model of the palliative care physician or nurse practitioner, and social worker, working together as a team, has proven quite successful in terms of patient support and continuity. These clinics have contributed to significant decreases in expenses from hospitalization and emergency room utilization, supporting the strong impetus of the health system to expand the use of outpatient palliative care.

We take a close look at this pilot and how it can be applied in many settings. We’ll examine:

  • A strong model of cooperative team work between physician and social worker which addresses psycho-social issues effectively
  • Patient data used to identify palliative care patients
  • Materials to educate primary care providers and patients about this care
  • Mechanics of operating palliative care clinics in the primary care setting
  • Types of patients seen
  • Quality measures
  • and outcomes in utilization
