About the Presentation
New innovative models of integrating palliative care into clinics and home-based programs are emerging. Services have become available in mostly oncology-related clinic settings, and more recently, hospitals and health systems are implementing new models of care whereby palliative care clinicians make visits to patients in their homes. This presentation will explain the rationale for establishing a home-based nurse practitioner staffed palliative medicine service and describe the clinical, logistical, and operational considerations involved in providing such a service.
